Therein, the WCJ made the following findings:            In Case No. ADJ1686177, the WCJ found that applicant sustained industrial injury to her bilateral upper extremities while employed on October 18, 2002 by Mario Del Fante Florist, insured by Employers Elite Insurance Company (Employers Insurance), causing temporary disability from October 18, 2002 to October 31, 2002, causing 15% permanent disability after apportionment, and causing a need for further medical treatment. The WCJ further found that , applicant did not sustain psychiatric injury on an industrial basis and that Employers Insurance was liable for Labor Code1 section 5814 penalties in the amount of $2,151.00 due to its unreasonable delay in the payment of benefits.            In Case No. ADJ1816809, the WCJ found that applicant sustained industrial injury to her right shoulder while employed on May 13, 20042 by Anthony Allbiso Enterprises, Inc., dba, Flowers by
