Based on our review of the record, and for the reasons stated in said report which we adopt and incorporate, we will deny reconsideration. To the extent that defendant is raising Labor Code section 4658(d)(3)(A), we note that applicant’s permanent disability award is not based on the formula set forth in the chart provided in section 4658(d)(1). Rather, it is based on the requirement of section 4659(b) which mandates that defendant pay applicant her temporary disability indemnity rate for life.
