By that decision, a workers’ compensation administrative law judge (WCJ) found in case number ADJI784I271, that applicant sustained an industrial injury to her right ankle, right hip. right knee, and back arising out of and occurring in the course of her employment by defendant as a teacher’s aide on October 16, 2000. The WCJ further found that the injury caused 32% permanent disability, payable in the total sum of $18,044.60. Our Opinion and Order Denying Petition for Reconsideration was based upon our review of the WCJ’s repon and recommendation on petition for reconsideration (Repon), which we adopted and incorporated. Following the issuance of our Opinion and Order Denying Petition for Reconsideration on April 30. 2010, the offices of the Appeals Board was contacted by the WCJ who issued the decision and authored the Repon. The WCJ informed the Appeals Board that although a signed version of his Report had been provided to us with the legacy case files, his office had inadvertently scanned an unsigned version of the Report into the Electronic Adjudication 1 Defendant has identified two case numbers in its petition. A DJ1784127 and ADJ390690I. However, the Findings and Award issued by the WCJ identified case number ADJ1784127 only. , Management System (CAMS). Thereafter, the WCJ’s office scanned a signed version of the Report in HAMS.
