The F&A states, “During the period from March 15, 2013 to June 22, 2016, Defendant paid Applicant $13,537.14 in permanent disability indemnity.” (Finding of Fact no. 3.) Defendant states in its Petition, “The Defendant would like to put the Board on notice that a mistake was made on the Pre- Trial Conference Statement which was then restated in both the Minutes of Hearing and Summary of Evidence and then in the Findings of Fact #3. The mistake is that Permanent Disability has been paid from March 13, 2015 to June 22, 2016 and not from March 15, 2013. These correct dates are shown in Defendant’s Exhibit A.” (Petition, p. 2:22-26.) In turn, defendant’s Exhibit A reflects a starting date of March 13, 2015. (Exhibit A, p. 1.) Accordingly, we will amend the F&A to reflect the correct starting date for payment of permanent disability indemnity benefits.
