Therein, the WCJ found that applicant sustained industrial injury to his cervical spine, lumber spine, bilateral upper extremities, left lower extremity, and “psyche” while employed as a machinist/forklift driver/truck driver during the period February 14, 1967 through August 15, 1994 (Case No. ADJ2790090) and sustained industrial injury to his cervical spine and bilateral upper extremities while employed as a forklift driver/truck driver on October 8, 1990 (Case No. ADJ2006433). The WCJ further found that applicant earnings at the time of injury were $627.29 per week entitling him to a temporary disability indemnity rate of $418.19; that the injury in Case No. ADJ2790090 (CT: 2/14/67 – 8/15/94) caused temporary disability from August 23, 1994 through February 26, 1995, September 17, 1997 through May 27, 1998, and October 1, 1998 through December 7, 1998, with credit for payments previously made, and caused 45% permanent disability after apportionment; and that the injury in Case No. ADJ2006433 (DOI: 10/8/1990) caused temporary disability from October 9, 1990 through April 8, 1991, with credit for payments previously made, and caused 7% permanent disability after apportionment. The WCJ found the following apportionment: orthopedic permanent disability is apportioned 28% to the injuries resolved by Stipulated Award in Case Nos. LB0164658 and LB00201523; 20% to non-industrial factors; “5/27 of the remaining permanent disability to the injury
