In that F&A, the WCJ awarded applicant’s attorney’s fees pursuant to the petition for attorney’s fees under Labor Code2 section 5710 and denied applicant’s attorney’s petition for sanctions, penalties, costs, and attorney’s fees under Sections’ 5813, 5814, and 5814.5 (petition for sanctions).            Applicant’s attorney contends that monetary sanctions and costs should have been awarded under Section 5813 and Rule 10561 for defendant’s and its attorney’s bad faith actions; and, penalties and attorney’s fees should have been awarded under Sections 5814 and 5814.5 for defendant’s failure to comply with the order of July 18, 2011 for attorney’s fees under Section 5710. 1 The petition for reconsideration does not identify the filing attorney in the heading, and the petition forreconsideration and the answer do not list a California State Bar number in the heading. We direct counsel forboth parties to Rule 10232, subdivision (4), which requires that all non-form legal pleadings contain a heading with the filing attorney’s name and California State Bar membership. (Cal. Code Regs., tit. 8, § 10232, subd. (4).) 2 Unless otherwise stated, all references are to the Labor Code. ,             We received an answer from defendant. We received a Report and Recommendation (Report) from the WCJ in response to the petition for reconsideration, which recommends denial of the petition.            We have reviewed the record and considered the allegations of the petition
