In that Order, the WCJ noted that lien claimant settled “all WCAB liens related in any way to any date of service through December 31, 2003” in exchange for payment of $325,000.00 payable by defendants in the consolidated matter of Luis Cardozo (Master File) ADJ1702197 (Settlement Agreement and Release of Claims II Terms of Settlement, p. 1). The WCJ’s Order noted that defendant, State Compensation Insurance Fund, and lien claimant settled all contested claims through December 31, 2003. In the Report and Recommendation on Petitions for Removal and Reconsideration (Report), the WCJ noted at page 2: “The parties appeared before the undersigned WCJ for lien trial on July 11, 2012. At the lien trial, Theodore Durden from Durden & Associates represented the interests of [Beverly Hills Center for Arthroscopic and Outpatient Surgery]. Dr. Hekmat, Dr. Durden’s contact for settlement authority, also attended the lien trial.
