County Of Sacramento, Permissibly Self-Insured Lorraine Rodgers , issued a Primary Treating Physician’s Permanent and Stationary Report (PR-4) giving the following medical history: “Failed initial treatment with PT and NSAIDs. 12/1/08 [sic] MRI showed large knee [effusion], large multiloculated Bakers cyst and Medial femoral condyle chondromalacia. 2/5/09 [sic] Dr. Hu performed 5 arthroscopic surgery reporting partial lateral meniscetomy, medial plica excision and removal of loose foreign body. Post op she attended PT but had not been 6 progressed. Second opinion by Dr. Coward to date has not suggested repeat surgery at this time.” (Dr. Wiggin’s 9/2/09 report, at p. 2, defendant’s Exhibit 7 A.) Dr. Wiggins gave impairment ratings of 1% WPI for the partial lateral meniscus and 3% WPI for the medial hyaline cartilage tear using Table 17-33 (page 546); 2% WPI using Table 17-31 (page 544); and an additional 2% WPI for pain. Thereafter, on January 7, 2010, applicant underwent an AME by Dr. Renbaum. Dr. Renbaum issued a January 28, 2010 report stating the following: “HISTORY OF INJURY “The patient had a course of conservative treatment including physical therapy and medications which provided no lasting improvement. “An MRI of the left knee was obtained on December 1, 2008, and showed a small medial femoral condyle hyaline cartilage tear with a large joint effusion, grade I medial collateral ligament sprain, and a small multiloculated Baker’s cyst. “She was seen by Dr. Hu for consultation. She had a cortisone injection on December 22, 2008, which provided no improvement. “On February 5, 2009 she was taken to surgery by Dr. Hu where she underwent arthroscopy, chondroplasty of the medial femoral condyle and lateral tibial plateau with partial lateral meniscectomy and medial plica excision with loose body removal. Postoperatively she attended physical therapy. “The patient states that she had no improvement in her left knee symptoms following recovery
