Applicant contends that the WCJ erred in setting the case for trial on May 21, 2014. In the alternative, applicant contends that he will suffer severe prejudice due to the WCJ setting the case for trial on May 21, 2014. We have not received an answer from defendant. The WCJ prepared a Report and Recommendation on Petition for Reconsideration (Report), recommending that the Petition for Reconsideration/Removal (Petition) be denied and noting that applicant’s failure to provide an Amended Application for Adjudication of Claim “is good cause for consideration of sanctions.” We have considered the Petition and the contents of the Report, and we have reviewed the record in this matter. For the reasons discussed below, we will dismiss applicant’s Petition for Reconsideration. Additionally, we will deny applicant’s Petition for Removal.FACTS While employed by defendant as a driver on July 19, 2012, applicant sustained an admitted industrial injury to his left index and left middle fingers. Applicant also claims to have sustained an industrial injury io his left shoulder, left arm, left elbow, left forearm, left hand, left wrist, bilateral upper , and lower extremities, back, neck, gastrointestinal system, and psyche, and in the form of high blood: pressure, irritable bowel syndrome, and sleep disorder.
