The WCJ also found that, while also employed by Addus as an LVN on April 22, 1997 (ADJ1958969), applicant sustained industrial injury to her low back. Finally, it was found that, while employed as an LVN by Alta Bates Medical Center (Alta Bates) on July 2, 2005 (ADJ3068271), applicant sustained industrial injury to her low back, left knee, bilateral lower extremities, bilateral feet, left shoulder, psyche, and in the forms of urinary and sexual functioning. As relevant to the instant petition for reconsideration, the WCJ found that “the 1997 low back injury [sustained while employed by Addus (ADJ1958969)] caused 40 percent of the need for medical treatment following the 2005 injury [sustained while employed by Alta Bates (ADJ3068271)], and the 2005 injury caused the balance.” Previously in these matters, by way of a stipulated award dated issued on December 15, 2000, it was found that the two 1997 injuries sustained while the applicant was employed by , Addus combined to cause permanent -disability of 33% and the need for further medical treatment.
