(LAO 0884582) is a case in which Jose Lugo, a truck driver for A. Rivera Trucking, sustained a crushed pelvis and left leg when an overloaded trailer backed up and hit him. He died on 2/8/08 as a result of his severe injuries. His wife, Aracely Lugo, filed an Application for Adjudication for the death claim.
