In that determination the workers’ compensation administrative law judge (WCJ) found in ADJ8867335, that applicant, James Ward, while employed as a rigging technician on November 10, 2011 by Entertainment Partners, insured by Insurance Company of the State of Pennsylvania, sustained an industrial injury to his low back, left ankle, left knee, hypertension, heartburn, IBS, sleep, and psyche. The WCJ also found in ADJ8867339, that applicant sustained an industrial cumulative trauma injury over the period February 23, 2002 through February 23, 2012, while employed by Entertainment Partners, NBC Universal, insured by Helmsman, and Cast & Crew, insured by Zurich North America, to his low back, thoracic spine, neck, both ankles, both knees, both wrists, both elbows, both shoulders, hypertension, heartburn, IBS, rhinitis, sleep, and psyche. The WCJ found that both injuries caused applicant to sustain permanent total disability, without apportionment, and awarded him permanent disability indemnity for life at the rate of $1,010.50 per week, increasing each January 1st commensurate with any increase in the State Average Weekly Wage, less credit for sums paid, and less an attorney fee of $279,329.44, to be commuted from the side of the award. Defendant Entertainment Partners, by and through its insurer Insurance Company of the State of Pennsylvania, was ordered to pay and administer the award, subject to contribution and reimbursement in subsequent proceeding
