GREEK ORTHODOX CHURCH; UTICA NATIONAL INSURANCE GROUP ION CIRSTEA WORKERS’ COMPENSATION APPEALS BOARDSTATE OF CALIFORNIAION CIRSTEA, Applicant,vs.GREEK ORTHODOX CHURCH; UTICA NATIONAL INSURANCE GROUP, Defendants.FLORYAN ASHLEY BUSSEY, Lien Claimant.Case No. ADJ194842 (SAC 0301466)OPINION AND DECISION AFTER RECONSIDERATION We previously granted the separate petitions of applicant and of defendant for reconsideration of the March 8, 2011 Findings and Award of the workers’ compensation administrative law judge (WCJ or WCALJ) who found based upon the parties’ earlier October 20, 2010 stipulations that applicant incurred industrial injury to his left hip, pelvis, back, head, shoulder and upper extremity and psyche while working for defendant as a maintenance man on February 24, 2001. The WCJ further found that applicant is temporarily disabled, that he is entitled to medical treatment in the form of a motorized mobility scooter, that he is entitled to reimbursement of medical mileage costs at $0.50/mile, that applicant’s Romanian interpreter Floryan Ashley Bussey is entitled to be paid in full on her lien claim, and that because applicant wrote a letter to psychiatric Agreed Medical Examiner (AME) Benjamin Carey, M.D., without sending defendant a copy of the letter a new psychiatric AME is required. Applicant, who is currently representing himself, contends that his letter to Dr. Carey was sent in good faith and without knowledge that defendant should receive a copy, and he requests that Dr. Carey be allowed to continue to act as the psychiatric AME./ / / , Defendant contends that the WCJ should not have found that applicant is temporarily disabled because temporary disability was not identified as an issue at trial and any temporary disability is due to applicant’s refusal of the psychiatric treatment proffered by defendant, that the motorized scooter should not have been allowed as medical treatment because defendant’s utilization r
