Based on our review of the record, and for the reasons stated in said report which we adopt and incorporate, we will deny reconsideration. In addition, we admonish the lien claimant, in future matters, to properly identify the person signing the petition on its behalf as well as the person signing the verification. The petition and verification were signed in cursive handwriting, but the signature is illegible and the printed handwriting below it, apparently one “Yecenia Mendez,” is not much better. Lien claimant is also advised to follow Division of Workers’ Compensation (DWC) rule 10232(a)(5), which requires all documents to be printed in “font of at least 12 points in size.” (Cal.
