This is our Opinion and Decision After Reconsideration. Defendants V Clad Laminates, Inc. and its insurers Pennsylvania Manufacturers Insurance Agency and Virginia Surety, administered by Illinois Midwest Insurance Agency (collectively “defendant”), petition for reconsideration of the April 23, 2012 Findings, Award and Order. In that decision, the workers’ compensation administrative law judge (WCJ) found, among other things, that substantial evidence supported applicant’s claim of industrial injury to the psyche and hands and allowed the liens of Psychological Assessment Services and Royal Medical Group. Applicant and defendant had previously settled applicant’s claim for admitted industrial injury to the left thumb and upper extremity and alleged industrial injury to the shoulders, wrists, hands and psyche on August 3, 2007 (ADJ1824746) and for alleged industrial injury to unspecified body parts from August 6, 2006 through August 17, 2007 (ADJ3300397), while applicant was employed as a machine operator/laborer for V Clad Laminates, insured by Virginia Surety from February 4, 2006 through February 4, 2007 and by Pennsylvania Manufacturers’ Association Insurance Company from February 4, 2007 through February 4, 2008. The WCJ awarded Psychological Assessment Services $22,772.71 as the reasonable value of the medical , treatment it provided, plus a 15% penalty and interest, and awarded Royal Medical Group “the reasonable value [of] the self-procured medical treatm
