Based on our review of the record and for the reasons stated in said report which we adopt and incorporate, we will grant reconsideration, amend the WCJ’s decision as recommended in her report, and otherwise affirm the decision of October 10, 2011. In addition, we are making certain other amendments, including naming the correct defendant (i.e., the insurance carrier, not the claims administrator) as having liability under the award.
