Having completed our review, we now issue our Decision After Reconsideration. Applicant, Catrina Williams, and lien claimant, Law Offices of George Slotnick, seek reconsideration of the Findings and Award, issued December 17, 2010, in which a workers’ compensation administrative law judge (WCJ) found- applicant sustained an industrial cumulative trauma injury to her neck and bilateral upper extremities over the period June 19, 1995 through July 24, 1996, while employed as a phone salesperson by Diversified Photo Supply, insured by Fremont Insurance Company, now in liquidation and whose covered claims are the responsibility of California Insurance Guarantee Association (CIGA). As a result of her industrial injury, the WCJ found applicant sustained 55.75% permanent disability, for which she was awarded indemnity totaling $53,082.50, and need for further medical treatment. Applicant was also awarded reimbursement for medical treatment and medical-legal expenses, in amounts to be adjusted. Applicant’s attorney and one prior attorney were awarded fees. The WCJ found applicant’s injury was not the predominate cause of her psychiatric injury, and that her claims for Fibromyalgia, Complex Region Pain syndrome and Reflex Sympathetic Dystrophy are included within a non-industrial Undifferentiated Somatoform Disorder vers
