We hereby issue a Notice of Intention to admit the report of Gary Halote (dated June 12, 2009) and the report of Dr. David Glaser (dated March 18, 2008) into evidence.            At the February 9, 2010 trial, the WCJ marked Applicant’s Exhibit 5 (Gary Halote, June 12, 2009 report) and Defendant’s Exhibit B (Dr. David Glaser, March 18, 2008 report) for identification only. It appears that defendant objected to Exhibit 5 and applicant objected to Exhibit B. After reviewing the file, it does not appear that these reports were ever admitted into evidence. Therefore, we will issue a Notice of Intention to Admit the reports of Gary Halote dated June 12, 2009 and Dr. David Glaser dated March 18, 2008 into evidence unless good cause to the contrary is shown in writing and is received within ten (10) days of service of this decision.            Upon expiration of the ten (10) day period, the matter will be deemed submitted for Decision After Reconsideration without further notice.            For the foregoing reasons,            NOTICE IS HEREBY GIVEN that the reports of Gary Halote dated June 12, 2009 and Dr. David Glaser dated March 18, 2008 will be admitted into evidence as Exhibit 5 and Exhibit B, respectively, ten (10) days after service of this decision, unless good cause to the contrary is shown in , writing and is received within that ten (10) day period at the Appeals Board, Attention: Commissioner’s Office at its street address, 455 Golden Gate Avenu
