This is our Opinion and Decision After Reconsideration. Defendant sought removal in response to and reconsideration of the Findings and Award (F&A) issued by a workers’ compensation administrative law judge (WCJ) on January 16, 2014. The WCJ found that applicant sustained industrial injury to his bilateral upper extremities, bilateral lower extremities and internal systems; that applicant was entitled to reimbursement for self-procured medical treatment and expenses, including a wheelchair, caregiver services by Kim Watt, home health care services by Martha Rios, home modifications, an adjustable bed, costs for a trip to Florida, and mileage; the issue of penalty or interest was deferred; and the issue of applicant’s attorney’s fees was deferred. Defendant contended that the WCJ should have found that care giver Halimah Shenghur was not entitled to payment for home health care services and that applicant was not entitled to reimbursement for services provided by Ryan Pinkham and Mark Leach; and that the WCJ erred in awarding applicant reimbursement for modifications to his home. We received an Answer from applicant. We received a Report and Recommendation on Petition for Reconsideration (Report) from the WCJ in response to defendant’s petition, which recommended that the Petition for Removal and for Reconsideration be denied, , We have reviewed the record and have considered the allegations of the Petition for Removal and for Reconsideration and the Answer and the
